Camper-Related Acknowledgements

I acknowledge on behalf of the campers herein the inherent risks of camp activities, including but not limited to physical activity, travel, and outdoor environments, and accept full responsibility for their participation.

I give permission on behalf of the campers herein to participate in all PCAFI CAN Camp activities, including worship, recreation, and educational programming.

I certify on behalf of the campers herein that each is physically and mentally able to participate in camp activities, and that all known medical conditions and medications have been disclosed in this registration.

I release PCAFI, its officers, staff, and volunteers from any liability for injury, illness, loss, or damage arising from the participation of the campers herein in CAN Camp activities.

I grant permission on behalf of the campers herein for PCAFI to use photographs and video recordings taken during CAN Camp for promotional, educational, and ministry purposes.

I acknowledge on behalf of the campers herein that each is expected to abide by the PCAFI CAN Camp Code of Conduct, and that failure to do so may result in dismissal from camp without refund.

I agree to complete Guardian Preparation Training as required by PCAFI prior to camp, and to fulfill all parental responsibilities in preparing the campers herein for a safe and meaningful CAN Camp experience.
